1) Log into the Federal Student Aid website at studentaid.gov , click on Aid Summary. View Grants and Loans breakdown. This information is necessary to complete this form.
2) Please list all educational institutions attended during the time frames listed on the back of this form. Using the studentaid.gov website, you will be able to view your Federal Financial Aid records associated with the schools you’ve attended. If you attended multiple schools during the indicated time frame, attach an additional page.
3) You must attach an academic transcript or grade report from each educational institution attended. You must submit an official academic transcript from each previously attended accredited college to the RSC Office of Admissions and Records if you have not already done so. Failure to report all colleges with attached transcripts/grade reports will result in denial of your request for Federal Financial Aid at RSC.

* If you answered “No” to the Credit/Clock Hours Earned question for any of the schools you listed on the back of this form, you must provide a typed detailed explanation of the special circumstance(s) that caused your failure to earn academic credit/hours at each school and provide third-party documentation to support your explanation. This office will review your academic transcripts, explanation and documentation to determine your financial aid eligibility.
